Proctor forms are due NO LATER THAN 2
weeks prior to the first day of final exams. See your
course homepage for the specific due date. Proctor approval is
a process that takes time and cannot be completed at the last
minute. If a student fails to submit a proctor form by
the due date, he or she will need to make arrangements to take the
exam at the college's campus in Barstow, Fort Irwin, or Miramar.
ATTENTION: This is a form to
be filled out by students who require proctoring for their
tests. If you are able to come to Barstow College
at Barstow, Fort Irwin, or the Miramar campuses you should do so, and you do
not need to fill out this form. (Students
who are close to Barstow College, Fort Irwin or Miramar will not be approved for
proctors except in extenuating circumstances - contact the instructor with
an explanation before submitting this form or your request will be denied.)
If you will need a proctor, complete the box by entering your name, email address,
postal address and phone number. Send the information to your instructor by clicking
on the "Submit Answers" button at the end of the proctor form.
Failure to follow all instructions below will result in having your proctor
rejected.
In addition, filling out this form
does not automatically mean your proctor has been accepted. The
proctor will still need to receive instructor approval. Proctors ARE
verified, and all students will receive notification of acceptance (or denial)
of their proctor no later than 1 week before the final exam is scheduled to
begin.
Barstow College
has specific requirements for a person to serve as a proctor.
Please be
sure your proctor meets these requirements BEFORE submitting the information
or your proctor will be denied.

All proctors must be librarians, military educational officers, college
professors, public school teachers (counselors/principals), professional proctors (such as our lab
aides or a library testing center), or clergy. (Working at a
public school is not enough qualification to be a proctor - the
person must be a certificated teacher, counselor, or principal.)
* The
proctor must have a professional email address and phone number.
(Hotmail, Yahoo!, Gmail, Excite, AOL, Netscape, EarthLink, Verizon,
Netzero,
etc., email accounts are all unacceptable and will be denied.)
A professional email address includes the person's workplace in the
suffix of the address. (NOTE: if the proctor form does
not contain a professional email address, it will automatically be
rejected. This is the most common reason that proctors are
rejected.) (also - be sure to double-check the email address -
it is not uncommon that it is typed incorrectly and doesn't work the
day the exam is sent out, leaving the student scrambling to find the
actual address. It's wise to just send an email to the proctor
yourself and make sure that it works before submitting it on the
proctor form.)
* The proctor must have a computer
with reliable internet access available in the supervised area for you to take
the exam. All exams are on the computer. Printed/paper exams are not
available. Exams cannot be given in the proctor's home - they
must be given at the professional place of business.
* Regardless of profession, friends, family members, and
neighbors are not suitable proctors.
When you submit your proctor
form, your instructor will email you within 48 hours to let you know
that the form was received (this is not an approval). If you
do not hear back from your instructor within 48 hours letting you
know that the form was received, please email the instructor to be
sure.
Your instructor will email you with
proctor approval no later than 1 week before the exam is scheduled to
begin. Proctor information is sent to proctors the evening before the
exam is scheduled to begin. It is wise for the student to verify that
the proctor has received this information prior to going to take the exam
(preferably ASAP) and to notify the instructor if the proctor has not received
the information.
Finally,
please remember that the top sections are for YOUR information and the bottom
sections for your PROCTOR'S information. Any costs
related to proctoring are to be born by the student. The college is
not responsible for proctoring fees. You must provide a picture ID to
